Out of State Physician Surrenders License   

OLYMPIA, WA – Steven Wayne Powell, MD (Lic. #61017983) voluntarily surrendered his license to practice after entering into a Stipulation to Informal Disposition with the Washington Medical Commission (WMC). This action was based on Powell surrendering his license to practice medicine in Louisiana.

The WMC alleged that while under investigation for unprofessional conduct, Dr. Powell entered into a Stipulation and Agreement for Voluntary Surrender of License with the Louisiana State Board of Medical Examiners.

Dr. Powell’s medical license has also been suspended, surrendered, or revoked in fourteen other states. Dr. Powell resides in the state of Georgia.
