DWC updates employer forms

DWC has revised:


* DWC Form-001 [ [link removed] ], "Employer's first report of injury or illness".
* DWC Form-002 [ [link removed] ], "Employer's report for reimbursement of voluntary payment".
* DWC Form-006 [ [link removed] ], "Supplemental report of injury".

These form revisions were necessary to update the forms to conform to our agency’s current form standards and use plain language. We also removed a reference to outdated violation language.

These forms are effective January 20, 2025.
