**The starting salary is 25295 (level 1) rising to 26999 (level 2) on first year anniversary, subject to satisfactory completion of a probation period (pro rata for part time)*

* We are looking to recruit a passionate, proactive and capable individual to undertake the duties of Children's Continuing Care Funding Coordinator, based within the Children's Placements Service. The team work closely with colleagues from children's social care to source suitable placements for children who are coming in to care or who need to move. Some of these children have needs beyond the average child in care and require additional support within their placement, to enable them to thrive. These may be physical, mental or emotional health needs. As the Continuing Care Funding Coordinator you will lead upon the completion, submission and review of referrals and assessments for Continuing Care funding. You will liaise with multi-agency professionals to inform the Continuing Care referral and assessment process. Your work will help ensure that Nottingham's children and young people are accessing the right support witin their placements to help them thrive in our care.
*ESSENTIAL REQUIREMENTS:*

* Ability to work proactively with Social Workers and health colleagues to undertake joint work, including quality assessments, in a timely way.
* Ability to prioritise own workload, plan work and track progress and meet deadlines as required
* Ability to ensure accuracy of report from ICT systems and spreadsheets
