If you or a member that you support has not taken the SIS-A assessment yet, there is still a SIS-A incentive available to take the SIS-A right now. But that incentive will end soon. There are less than 50 incentives remaining. If you would like to receive the incentive for the SIS-A assessment, you should plan to contact Maximus soon.
For background, the SIS-A is high-quality data that helps inform the Person-Centered Planning process. It also provides OADS with support information for the design of innovative service options through the new Lifespan Waiver.
Also - OADS has expanded who can take the assessment to include youth aged 16 or older.
