Each video has practical tips and examples created and narrated by someone who has been self-directing for over 20 years.
The Individual-Directed Goods and Services Request Form has been updated. The member or Representative must complete this form for each item on the spending plan. The additional questions will help ensure the purchase is allowed and the request is tied back to the PCP.
