Pierce County Physician Restricted

OLYMPIA, WA – In June 2024, the Washington Medical Commission (WMC) entered into an agreed order with Fadi Alhafez, MD (Lic. #60094946) restricting his medical license. 

The WMC alleged that Dr. Alhafez committed unprofessional conduct by improperly performing a cosmetic surgery procedure and improperly prescribing testosterone replacement therapy. 

The agreed order prevents Dr. Alhafez from independently performing cosmetic surgery until he completes 20 supervised cosmetic surgery procedures. He must also undergo monthly reviews of his hormone replacement practice, complete continuing medical education, write a paper, and pay a fine of $20,000.
