The Office of Aging and Disability Services has made some changes to the previously known ?Vendor Call? process. The new forms are now electronic, and you will receive an email with your submission details.
Below are the links to the new electronic forms:
Providers that have capacity should contact the listed Case Manager or Care Coordinator via email with the subject line ?Responding to Service Request.? Case Managers and Care Coordinators should reply to Providers within three (3) business days.
If the member's service request changes or once they have started services, please complete the form again with the updated information.
