Connecting Resources & Driving Equity: Opportunities to Build and Spread Sustainable Community Referral Networks

Community Referral Networks ([link removed]) (CRNs) are critical care planning systems to help community members access essential resources. Our Research to Pursue and Advance Racial Equity (RePARE) project looked into the design and sustainable support of CRNs to enhance both community health and racial equity. This report shares challenges and opportunities for expanding the role and impact of CRNs in enabling healthy communities.
