DWC revises supplemental income benefits form

DWC has revised DWC Form-052 [ [link removed] ], "Supplemental income benefits (SIBs) application", effective June 30, 2024.

The DWC Form-052 is necessary for injured employees to apply for SIBs. The revisions were made to make the form more user-friendly and to clarify work search requirements.
