MCN & NNCC Learning Collaborative:
The Road to Patient Activation
Patient engagement is increasingly recognized as an essential component of high-quality person-centered healthcare. Evidence-based research shows that engaged patients are able to make more informed decisions about their own health which promotes mutual accountability and leads to more efficient use of scarce health resources. While there is little disagreement about the value of an engaged patient population, in practice it can be challenging for health centers to design interventions and policies that facilitate full engagement.

In this six-part interactive series, Migrant Clinicians Network (MCN), the Institute for Patient and Family Centered Care (IPFCC), and National Nurse-Led Care Consortium (NNCC) will work with a select group of ten health center care teams to improve knowledge and skills around evidence based strategies for patient engagement and activation. Participants will explore a continuum of engagement that examines individual interactions, institutional design, and larger policy design implications.
The series will begin
