Do You or Your Child Have Asthma?

We invite you to our annual asthma needs assessment if you live in Los Angeles or San Bernardino County! This anonymous survey should only take 10-15 minutes.

Its primary objective is to identify and prioritize the unmet health needs of children and adults diagnosed with asthma in our communities, laying the groundwork for future initiatives to address asthma-related health challenges.

While completing the survey, participants with asthma who include their email addresses will be entered to WIN an ATMOtube PRO Air Sensor!
