Here are some simple ways to spread the word: Display this poster at your school to celebrate you, the leader, optimizing student health, safety, and learning. Let's make some noise for #SND2024! By sharing these messages and images on social media, you can honor school nurses and join this nationwide celebration. Your voice matters! Support from a mayor or governor can raise community awareness. You can use the National Proclamation to request public recognition from local government officials. These knowledge-sharing conversations are a great way to connect and learn from new people with similar interests.   Managing Chronic Health Conditions Transition Planning for Students with Healthcare Needs The continuation of safe, quality, equitable care for all students with healthcare needs is essential for supporting positive health and academic outcomes so that all students have equal opportunities to attain their full potential. NASN believes that healthcare transition (HCT) planning and implementation for all students with healthcare needs should be a well-coordinated process that occurs whenever a student moves into, through, or out of a school setting. As a vital member of the interprofessional school-based team, school nurses are ideally situated to provide expertise, leadership, and care coordination for students during HCT. NASN is pleased to offer two no-cost NCPD programs and a toolkit to help you understand your role in HCT coordination for students with lupus and epilepsy.
