SASD Support Team to discontinue fax number [ [link removed] ]

* *Audience:* Lead agency and provider staff who enter data in the Medicaid Management Information System (MMIS) for home care, Alternative Care, Essential Community Supports or waiver service agreements/screening documents
* *Effective date:* May 1, 2024
* *Summary:* Effective May 1, 2024, the DHS SASD Support Team will no longer have an active fax number. DHS will not receive forms sent to the discontinued SASD fax number.

* *Audience:* Organizations that work with new Americans, LTC providers, workforce and employment organizations and other interested parties
