School nurse-led case management (SNLCM) assists students with health-related needs to manage their needs successfully, reduces barriers to academic performance, and increases student engagement. NASN's manual, A Model for School Nurse-Led Case Management, describes SNLCM and its benefits for students. It helps guide school nurses and school nurse leaders by providing implementation and evaluation strategies and tools.
