Think back: It was a snowy day in January 2022 as vehicles whizzed across the Fern Hollow Bridge. 

Then, the bridge collapsed. 

For two years, federal investigators have been probing the collapse's cause.

On Wednesday, the National Transportation Safety Board approved a report that revealed the collapse was the result of Pittsburgh ignoring the warnings by inspectors for a decade.
