* *Summary:* As of Nov. 1, 2023, DHS implemented a new long-term services and supports (LTSS) remote reassessment policy and resumed face-to-face visit requirements for case management and care coordination.

PSN form now available for MHM eligibility and enrollment process [ [link removed] ]

* *Audience:*?MHM?transition coordinators, lead agency staff, case managers, substance use disorder (SUD) treatment programs, care coordinators and other interested parties
* *Effective date:*?Jan. 9, 2024
* *Summary:* Qualified professionals and people seeking MHM transition services can now use the PSN form to meet the screening document requirement in MHM?s eligibility and enrollment process.

Disability Services seeks Innovation Grants applications [ [link removed] ]

* *Audience:*?All interested parties
* *Summary:*?DSD seeks grant proposals to support innovative strategies to improve outcomes for people with disabilities. We will give priority to proposals that serve people eligible for or who receive home and community-based services. The 2024 Innovation Grants request for proposal includes two programs: the Innovation Small Grant Program (grants up to $60,000 per year) and the Innovation Large Grant Program (grants greater than $60,000 per year).
