Medicaid Waivers

Two Washington state Medicaid waivers will continue for five more years.

The Centers for Medicare and Medicaid Services (CMS) approved the renewal of two of our state?s 1915(c) waivers, the Community Options Program Entry System (COPES) waiver and the Residential Support Waiver (RSW).

COPES and RSW renewals are effective January 1, 2024 through December 31, 2028.

What is the COPES waiver and the RSW?

COPES

The COPES waiver was implemented in 1982 and is one of the oldest waivers in the nation. It provides services to over 53,000 individuals living in their own homes, Assisted Living Facilities, and Adult Family Homes in Washington state. Without the home and community-based services and supports provided under COPES, individuals would require the level of care furnished in a nursing facility. Services in the COPES waiver act as a wraparound to service available to the Community First Choice (CFC) State Plan program. Services available in the waiver include:

  • Client Support Training/Wellness Education
  • Skilled Nursing
  • Home Delivered Meals
  • Environmental Modifications
  • Specialized Medical Equipment and Supplies
  • Transportation
  • Adult Day Care
  • Adult Day Health
  • Community Choice Guiding
  • Community Support: Goods and Services

RSW

As an important element of Washington State's commitment to provide community alternatives to institutional care, the RSW began in 2014 and provides supports and services in licensed and contracted community residential settings (Adult Family Homes, Assisted Living Facilities, and Enhanced Services Facilities) to adults who are eligible for nursing facility level of care and have personal care and behavioral support needs. Services available in the waiver include:

  • Adult Day Health
  • Behavior Support
  • Client Support Training and Wellness Education
  • Nurse Delegation
  • Personal Care Services
  • Skilled Nursing
  • Specialized Medical Equipment and Supplies

What changes were made to the COPES waiver and RSW?

COPES

  • Updated the maximum number of participants authorized to be served in the waiver each year.
  • Added a new provider type, Assistive Technology Professional, to the Client Training and Wellness Education service.
  • Updated Performance Measures.

RSW

  • Updated the maximum number of participants authorized to be served in the waiver each year.
  • Added a new provider type, Assistive Technology Professional, to the Client Training and Wellness Education service.
  • Updated Performance Measures.
  • Added Institutes for Mental Disease (IMD) and Inpatient Behavioral Health Agencies as eligible locations ALTSA will pay for retainer payments.
  • Added Assisted Living Facilities as a provider type for the Community Stability Supports (CSS) service.
  • Removed reserve capacity.
