This survey request was emailed to me from Nikki Heiman, teacher and member of the Employment First Oversight Commission.? All questions need to be sent to her [email protected].?

My name is Nikki Heiman and I am a Doctoral Candidate at Kansas State University. I?m sending you this email because I?m asking for your help in researching the current quality of life for young adults with IDD in Kansas.

This study is a 2-part study. Phase 1 is the survey attached to this email. If you would like to be anonymous, you can choose to not leave contact information. If you would like the opportunity to be selected for phase 2 and tell stories of your experiences, please leave your email address at the end of the survey. If you leave your email, you will also receive the results of the study when it is complete.?

Phase 1 will take around 15 minutes to complete. If you are selected to participate in phase 2, you will participate in an interview with the researcher that may take around an hour. This may take place at a location of your choice, or over zoom, depending on our shared schedules and locations.?

You will have the opportunity to help teachers understand important things about your lives, both during school, and how those experiences impacted your life after you graduated.?
