To assure care coordination for mobile patients, Migrant Clinicians Network's (MCN) Health Network program offers continuity of care and treatment completion by providing comprehensive bridge case management, medical records transfer, and follow-up services for mobile patients. This follow-up webinar to the Health Network Care Coordination Program for Mobile Patients discusses how to use the (MCNs) Portal for enrolling patients, uploading documents, and checking on status of previously uploaded documents.

Saul Delgado graduated high school in 2009. After graduating, Delgado became a Truck Dispatcher for his father’s company before deciding that he wanted to pursue a career in medicine, inspired by his mom, who was a nurse in Cuba. In 2011, Saul attended Southern Careers Institute where he graduated as a Medical Assistant. He worked as a Medical Assistant Floater in the Pulmonary, Cardiology, and Nephrology department in Austin Diagnostic Clinic. In 2012, Delgado joined Migrant Clinicians Network as a Health Network Associate. He has since taken on significant technology responsibilities, developing, and tailoring the Health Network database to best serve Health Network Associates in their work.
