*There are two positions available to work 30h/ week at Spring Meadow Day Centre. Roles are fixed term until July 2020 but may be extended.*

* The starting salary is 18,426 (Level One) rising to 18,795 (Level Two) on the first year anniversary, subject to satisfactory completion of a probation period (pro rata for part time workers). This equates to 9.55/hour.*

*Job Purpose*

Spring Meadow Day Centre is a modern day service in The Meadows for citizens with a range of learning disabilities and additional needs. The team are committed to providing safe and appropriate personalised care and support to citizens in a way that maintains their dignity, maximises independence and gives them choice and control over the way their care is provided. There is a focus on creative arts and performance, with the Evergreen theatre group being based here.

Care Workers at Level 1 implement Care Plans and report any concerns or reasons care plans cannot be followed, as well as monitoring care and welfare of citizens, recording accurate information on running records, communication books, care plans monitoring charts (e.g. weight monitoring, falls, etc.) and other written and electronic communications as required.

Care Workers at Level 1 are also expected to work and cooperate with other professionals, as well as citizens and their meaningful others, to ensure a shared approach to care and support and continuity of care and treatment.



*ESSENTIAL REQUIREMENTS:*


* Implementation of Care Plans and contribution to running records and daily monitoring forms;
* Ability to encourage and support citizens to achieve and/or maintain maximum independence, and to maintain contact with friends, family and community;
* Knowledge and understanding of the rights, needs and feelings of vulnerable adults from diverse social and cultural backgrounds;
* Ability to follow all instructions, policies, procedures and guidelines.

Level 2 workers must be able to work with a degree of independence and initiative, and be able to respond to urgent situations calmly and efficiently. *ESSENTIAL REQUIREMENTS:*
* Ability to assess social care and health needs and develop, implement, maintain and update Person Centred Care Plans;
* Experience of, or ability to, provide support and guidance to colleagues and to be able to deal with emergency and difficult situations;
* Knowledge and understanding of the rights, needs and feelings of vulnerable adults from diverse social and cultural backgrounds;
* Ability to administer medication in line with departmental policies and procedures.
