Advancing Health Equity in Algorithms

In their article in the October 2023 issue of Health Affairs, Michael
Cary Jr. and his team of coauthors offer a scoping review of literature
published between 2011-22 to identify tools aimed at mitigating racial
and ethnic bias in clinical algorithms
.

Identifying more than 100 articles, the authors identify a range of
techniques and strategies for bias mitigation, but no consensus on a
single best practice.

They broadly categorize mitigation strategies into four categories:
algorithm design, preprocessing, processing, and postprocessing.

The authors also find some gaps in the literature for addressing bias,
noting that "only five [studies] identified health equity as a key
component, and only three tested frameworks in real-world clinical
settings."

Cary and coauthors conclude with a series of recommendations for bias
mitigation in clinical algorithms including: ensure professional
diversity, require auditable clinical algorithms, foster transparent
organizational culture, and implement health equity by design, among
other recommendations.
