Thank you for your willingness to assist with the effort to get Mike on the ballot.

In order to make it, Mike has to get 500 signatures in each congressional district and he needs your help to do it!

Here’s how to help:

Download your ballot petition here then print it out.

The form can be completed by any registered Indiana voter in black or blue pen with the following information:

• Signature (must match the signature on their voter registration)
• Printed Name
• Date of Birth
• Residential Address (cannot be a PO Box, but must be your voting address)
• City AND Zip (must list both city/town name and zip code)

Here are some tips and important information:

✔️ Ensure that all of the candidate information is populated and the correct county is written at the top of the form. 
 
✔️ Any registered voter can sign the petition, just make sure that they fill in all information accurately and completely. 
 
✔️ Signing the petition does not necessarily mean that they will be supporting Mike, most folks will agree that competition is a good thing and voters deserve a choice. 

Once a form is completed, please enter your information in the Carrier Information Section and sign. This signature is attesting that none of the signatures or information was entered by you. 
 
The form should be physically returned to a member of the Braun team or mailed to PO Box 1126, Carmel, IN 46082.
