Please use this revised Medical and Vaccination Records Objection of Disclosure Letter when submitting pre-2019 issued Medical Exemptions to schools.

In October 2019, A Voice for Choice Advocacy shared an updated Objection to Disclosure of Student xxxxxxrmation and Records document, which was put together by Leigh Dundas of Advocates for Physician’s Rights.  In the past few months, as parents have submitted this to schools, we have gotten feedback that it is too long and complex, and that schools have refused to accept it.  Therefore our team has simplified this letter, while keeping its message strong.  If you are submitting a medical exemption, please attach this document to it to deter schools and school districts from sharing your medical records.  If you have already submitted a previous version to your school there is no need to update it with this one, unless you want to.  Please note: this documents is not 100% failsafe.  For example, if there is a court subpoena or an audit due to an emergency, the schools may still share the records, but it will protect your child’s records for the most part.
