Launched in 2017, the purpose of the AHC Model is to "test the impact of identifying and addressing Medicare and Medicaid beneficiaries’ health-related social needs on health care use and costs."
Renaud and coauthors administered a survey to AHC Model beneficiaries eligible for navigation services that connect community service providers with social services and assessed whether beneficiaries’ health-related social needs were resolved through the program’s navigation services.
Ultimately, while the AHC Model effectively engaged participants, with more than 75 percent of navigation-eligible beneficiaries opting to receive navigation, the authors find that "the assistance-track intervention did not significantly increase the rate of community service provider connection or the rate of health-related social needs resolution…"
These results suggest that AHC navigators need increased resources and capacity to address beneficiaries’ needs.
