Adult Social Care
? ?
The department is underpinned by the Better Lives Better Outcomes strategy, which aims to maximise and maintain independence for older and disabled citizens through community support where possible.
? ?
?We will enable all older and disabled citizens, including those with mental health needs, in Nottingham to live as independently as they can, with a connection to their communities.
??
When formal care and support is needed, its aim will be to retain and restore independence. No one will live in residential care unless all other options are exhausted.?
??
The Better Lives Better Outcomes strategy is based on these four themes:
??
Prevention: promoting healthy lifestyles and intervening early when people?s wellbeing is at risk, to avoid crisis and loss of independence.
??
Community Connections: ensuring citizens are connected to the resources and support in their local neighbourhoods.
??
Independent Lives: supporting personal and community resilience, strengths and resources, reducing dependence on council funded support where possible.
??
Choice and Control: seeing the citizen in the driving seat, shaping solutions around the outcomes which matter for individuals.
??
The department works in close partnership with colleagues in Health, Community Protection and local charities to implement a multi-agency approach to care and support. Social Workers are responsible for ensuring statutory duties under the Care Act, including the delivery of appropriate services to safeguard vulnerable adults and their families, according to their assessed needs. This includes undertaking all levels of assessments, creating and implementing care plans and support services, and case management. Social Workers are also expected to deal with risk management on a daily basis, sometimes under the supervision of a senior member of the team.
??
All Social Workers
??
Nottingham City Council Adult Social Care offers a robust ASYE programme for newly qualified Social Workers, and other various development programmes for all Social Workers, such as AMHP qualifications and Practice Educator status. All permanent Social Workers are given the opportunity to rotate into different areas of Adult Social Care, and take part in project work at a strategic level when available.
??
