* *The starting salary is 19,171 rising to 19,945 on first year anniversary, subject to satisfactory completion of a probation period. This is pro rata for part time hours.
* *Hours will be worked between 4:00pm and 11:00pm according to a rota, and this will include weekend work.
* *A valid driving licence and access to your own vehicle is preferred for this role, although we do also have some e-bike training opportunities for successful candidates.
* *Nottingham City Council Homecare provides two services; Social Care Reablement and JackDawe.
* Social Care Reablement is a specialist Homecare team that works across the city to provide support to citizens in their own homes. This is mostly acute, short term care for citizens who have recently been discharged from Hospital or another service. The focus of the work done by Social Care Reablement is to help citizens reintegrate back into their previous lives, whether that is socially, physically or emotionally.
The JackDawe team provide a specialist service supporting citizens with a range of complex needs, to enable them to continue with everyday activities and tasks that are meaningful to them. This also encourages independence and can reduce the impact of behavioural and psychological symptoms associated many complex needs. JackDawe service also supports in practical ways, such as medication management, personal care and home management.
Both teams encourage independence and choice for citizens, and control over the way their care is provided. This involves not only providing personalised care as directed in the citizens care plan, but also encouraging and enhancing the wellbeing of citizens through promotion of independence, early intervention, community support and friendships.
*Essential Requirements*
* Ability to assess social care and health needs and develop, implement, maintain and update Person Centred Care Plans;
* Experience of, or ability to, provide support and guidance to colleagues and to be able to deal with emergency and difficult situations;
* Knowledge and understanding of the rights, needs and feelings of vulnerable adults from diverse social and cultural backgrounds;
* Ability to administer medication in line with departmental policies and procedures.
