NHS England has refused to sign off the spending expectations for local systems, after a second round of financial plans warned of a combined deficit of £3bn, HSJ has learned.
The latest submissions from integrated care systems were supposed to be final, but the regulator is now working individually with some to try to reduce the overspend.
As previously revealed, the first round of submissions equated to a £6bn deficit, which has now been cut in half after ICSs were told to find more savings.
Officially, ICSs are tasked with delivering a balanced budget.
Several local sources said they are concerned that systems are being pressured into submitting unrealistic plans, with efficiency savings that cannot be delivered.
According to another trust’s board report, prepared before the final plan submission, trusts in the North East and Yorkshire were planning efficiency savings as high as 7 per cent of turnover.
One ICS leader described the numbers as “fantasy” and added: “How anybody is going to do 7 per cent without hitting patient care is beyond me. This isn’t planning, it’s just making figures up.”
A chief financial officer in another system said: “We’re into numbers now that are not doable by just efficiency, they have to be cuts.”
Ahead of last year’s autumn statement, NHSE warned it faced a budget shortfall of around £7bn in 2023-24.
The chancellor provided roughly half of this, £3.3bn, which Amanda Pritchard said at the time “should provide sufficient funding for the NHS to fulfil its key priorities”.
HSJ asked NHSE whether it would provide extra financial support. A spokeswoman said: “Work continues with systems to finalise financially balanced and ambitious plans for 2023-24 to deliver the best possible outcomes for patients.”

** Premier League bans gambling sponsorship on front of club shirts
The Premier League has banned clubs from having gambling sponsorship on the front of their match-day shirts.
There are eight Premier League clubs with gambling companies as shirt-front sponsors, with an estimated value of £60m each year.
Gambling sponsorship will be axed at the end of the 2025/26 season, the league said in a statement.
Until then existing - and new - shirt-front gambling sponsorship contracts will be allowed.
Clubs will continue to be able to feature gambling brands in other areas, including shirt-sleeves and LED advertising.
Culture Secretary Lucy Frazer tweeted: "While the vast majority of adults enjoy gambling without harm, we can't ignore the fact footballers are massive role models to kids.
"Our upcoming Gambling White Paper will upgrade punter protections & do more to protect those at risk of addiction."

** Australia: Ditch tobacco vending machines in pubs, lawmakers told
Tobacco vending machines in Queensland pubs and clubs are either "anonymous, drab units" or potential contributors to smoking relapse, a state parliamentary inquiry has been told.
Under a bill tabled by the state government last month, venue staff would have to retrieve cigarettes from a vending machine housed in an area not accessible by patrons.
Canberra bans vending machines entirely, Tasmania has a similar restriction and tobacco vending machines in NSW and South Australia must have a staff intervention mechanism.
There's evidence to suggest the presence of vending machines can trigger unplanned purchases of tobacco and undermine attempts to quit, committee members were told on Friday.
"A prohibition on vending machines will help to further de-normalise the use of smoking products and eliminate a strong visual cue which has the potential to trigger smoking relapse," Cancer Council senior manager Sharyn Chin Fat said.
Queensland Health executive director Mark West said vending machines offered ease of access to tobacco, which could influence relapse and increased consumption among social smokers.
"Given the strong association between alcohol and tobacco consumption, the presence of the machines in liquor licensed premises only exacerbates these risks," he said.

This will include one million smokers being provided a vape starter kit in a ‘swap to stop’ scheme and financial incentives for pregnant women to stop smoking.
